To help you through the process, we’re providing the steps and forms you’ll need based on your specific situation. Complete the appropriate forms(s) and submit them on our Distributed Generation Support Site. Please note that Notice of Assignment requires signatures of prior owner and new owner. If you require guidance on how to complete forms, refer to How to Complete a Schedule Z.

If you are a participant in the supplemental MA SMART program, additional documents are required and will be sent to you via secure email.  Please Contact Us to request these documents via DG inquiry.
