Smart Meter Opt-Out Authorization Form
We have received your intent to opt out of our smart meter program. In order to complete the opt out process, we kindly ask that you fill out this form, sign it, and send it back to us within 30-days of receipt.
Once you have opted out of the program, a new conventional or non-communicating meter will be installed.. Since this meter will not transmit data to us automatically, it will need to be read manually by a utility representative, and you will be charged the fees for this service as shown in the chart below. The fees cover fuel and vehicle costs, as well as additional field resources.
By opting out and signing this document, you, as the account holder, are agreeing to the additional monthly fees associated with non-communicating meters.

The one-time fee(s) will appear on your first bill after the non-communicating meter(s) has (have) been installed at your premises. The monthly manual meter reading fee will begin on the same bill. If you later choose to have a smart meter reinstalled, a one-time reinstallation fee will apply that is equal to the one-time change-out fee. Once your smart meter is installed, the monthly fee will be removed from your next bill.
