Smart Meter Opt-Out Authorization Form

We have received your intent to opt out of our smart meter program. To finalize the opt-out process, please complete and submit this form.

Once you have opted out of the program, a new conventional or non-communicating meter will be installed. Since this meter will not transmit data to us automatically, it will need to be read manually by a utility representative, and you will be charged the fees for this service as shown in the chart below.

By signing and submitting this form to opt out of a smart meter, you, as the account holder, acknowledge and agree to the associated fees. If this form is not completed and submitted, your request cannot be processed and a smart meter will be installed.

The one time change-out fee is charged if a smart meter has already been installed and is being replaced with a manual meter.
